We are not contracted with individual insurances; however, we work with a 3rd party biller Lactation Network. Lactation Network verifies your benefits, and gets approval prior to the appointment. They handle all the paperwork and bill your insurance directly. The most common plans eligible are UHC and BCBS.

*A $20 travel fee will be added for mileage over 10 miles from the 85233 zip code*

If cash pay, payment or payment arrangements must be made prior to visit being started. You will be supplied a superbill containing codes for you to submit for reimbursement, I will assist you in any way I can in getting the best possible outcome for insurance reimbursement. 

I accept cash, check, credit through square, and HSA cards through square.
