Insurance

Currently the only insurance we are able to bill is Cigna. We will supply you with a superbill to submit to your insurance for reimbursement, and assist in any way we can for you to get reimbursed.

*A $25 travel fee will be added for mileage over 10 miles from the 85233 zip code*

If cash pay, payment or payment arrangements must be made prior to visit being started. 

We accept cash, check, credit through square, as well as HSA cards through square.
